Care to Care and Premier (together, “CtC”) maintain proprietary Clinical Review Criteria (the “Criteria”) to ensure radiology related utilization management decisions are objective and based on current scientific evidence and expert opinion. You acknowledge that your access to the Criteria is subject to the following terms:

1. I do not have any right to claim ownership of the Criteria or use the Criteria for any commercial purposes. This includes, but is not limited to, the right to redistribute, sell, reproduce, license, lease, or engage in any other action to similarly transfer or transmit any portion of the Criteria. I am accessing the Criteria only in relation to my association with CtC as a provider of services to a health plan or similar type of coverage that I am: (i) currently enrolled in; (ii) am considering enrolling in; (iii) was previously enrolled in; or (iv) providing services to as a contracted health care provider.
2. I understand that the Criteria are not medical advice. The Criteria are not a substitute for treatment by a licensed medical professional and do not guarantee results or individual outcomes.
3. I understand that the Criteria describe medical necessity and use other technical terminology, which may not be applicable to a specific benefit determination or coverage decision made under my (or my patient’s) plan. I understand that my (or my patient’s) health plan may not cover medically necessary services, as described in the Criteria. Further, I understand that my (or my patient’s) benefit plan document (and not the Criteria) governs if a service is eligible to be considered a covered benefit, whether or not such service is contained, described, otherwise referenced in, or omitted from the Criteria.
4. I understand that state and federal laws, regulations, and similar government directives may affect the use or interpretation of the Criteria.
5. I understand that CtC is not responsible for any consequences or liabilities that relate to my use, misuse, interpretation of or reliance upon the Criteria.
6. I understand that the Criteria are updated regularly, subject to change, and the latest version may not be available or presented when accessed on the internet.
